This would enable me to formulate a working diagnosis that would indicate your treatment needs. Should you present with symptoms that fall outside my scope of practice I would refer you to the appropriate resources. No treatment shall be provided during this initial consultation or evaluation unless you choose to proceed with the therapy that I recommend. I'm contracted with blueshield and my rates are $150 for the first visit and $100 for follow-up visits. I currently only accept cash or check as payment.
